The Wound Detail of 4 Stages:

Stage Ⅰ Skin remains intact with persistent redness that does not fade when pressed. No open wound or blister is present on the heel.
Stage Ⅱ Patial-thickness skin loss presenting as a shallow open sore or intact blister. The wound bed appears pink and moist, with no dead tissue.
Stage Ⅲ Full-thickness skin loss extending into the fat layer. The ulcer may develop tunneling or undermining beneath the wound edges.
Stage Ⅳ Full-thickness tissue loss with exposure of muscle, tendon, or bone. Dead tissue or slough may be present within the wound.
